Respirator Fit Test – February 13, 2023
N.C. Cooperative Extension, Hertford County Center
9:00 a.m. – 2:00 p.m. (1:40 p.m. last time slot)
Fit Testing Fee: $25 per person | Medical Clearance: $25 | For Both: $45
Respirator Medical Clearance Form (recommended to complete prior to appointment but will be available upon arrival)

To be fit tested, please make sure to be clean shaven and to bring the following:
- A clean respirator if you already own one.
- Clean and unused cartridges (if you are using any other brand besides 3M or North Honeywell).
- Medical clearance documentation (medical clearance available on site)
- If your doctor is providing the medical clearance, please make sure it is on a letterhead.
- If you would like to be medically cleared at the fit-test event it will cost an extra $25.
